This is a great article (notwithstanding my other comment ;) ). I really like the way you express the differences between outcomes and systems and why both are important.

One more useful comment - don't get too hung up on it being exactly Five "whys", you might need fewer or more to get to the root and it is always useful to see if asking one more why gets you anything when you think you have reached that root.
