This does not gel with my experience. I have done separate consulting jobs in two different departments of an infrastructure organization that, by the nature of its work, tended to a safety culture - engineering and IT. If something went wrong in the engineering department, the first question was how can we fix it and the second was how can we prevent it from going wrong in the future. In the IT department, the first question was whose fault is it, then how can we fix it. And I don't think I ever heard how can we prevent it from happening again asked.
