I have only once been asked to do an exit interview. I declined despite heavy pressure from HR. I asked what would happen as a result of it (I actually had some constructive ideas) and was told that it would be filed for managers to reference if they wanted. But there was no formal follow-up or action as part of the process. So I said there's no point and continued to decline.
