A corollary to this is that people do not learn from success. If you get a good result, clearly what you did and the way you did it must be right ;). On the other hand, if you have a failure, you have two choices. First is find out who made the mistake and fire them. Second is find out what went wrong, how it went wrong and how to stop it happening in the future. I know which has the better results. A wise person once said processes are the scar tissue of our mistakes and that really stuck with me.
